We've decided on treatment

We got a second opinion yesterday, and we are going with that vet.

He was very patient and professional in sorting out the mess the other vets created. Charlie had been prescribed minocycline, which is an antibiotic! He took them from me and we threw them away. The doc gave us a DVD to borrow and learn about his condition, gave us statistics of how many cases he treats on average, showed us where the injections would happen and even told Charlie he was a good dog. (Acknowledging the owner and patient in the room! Whoa! Good job, doc!)

Treatment begins on Monday, they will observe him until I get him in the afternoon, I will bring him home and watch him through the night, and repeat that on Tuesday. He will have to be kept very quiet and calm for 3 weeks, off duty (because his back will be sore from the injections). In April he will be retested for heart worms (price included in Mondays treatment).

We found a good vet.
